Jack Johnson is a famous example of a fighter who turned great defense into great offence, throwing a punch with the same hand he would use for a block.
What other fighters were really good at turning defence into offence?

I'd say Bernard Hopkins and Jersey Joe Walcott could do it with footwork, taking a defencive stance or retreat and turning it into a beautiful attacking opportunity.
Floyd Mayweather can do it. He can pot shot from a heavy lean and turn a dip into a shooting opportunity.
